The Communicable Disease PCN will:

• Harness clinical expertise, leadership, public health knowledge, and patient experience to improve outcomes related to communicable diseases, including sexually transmitted and blood-borne infections (STBBIs).
• Transform evidence into practice and reduce variation in communicable disease prevention, surveillance, investigation, treatment, and outbreak response through engagement of clinicians, public health professionals, and care teams across Saskatchewan.
• Improve communication and coordination by connecting local teams, provincial programs, and system partners to share leading practices and align priorities.
• Accelerate innovation through partnerships and engagement with stakeholders across the health system to identify opportunities, test new approaches, and rapidly spread effective practices.
• Advance health equity by supporting access to consistent, evidence-informed communicable disease services across urban, rural, remote, and Indigenous communities.
• Foster a learning health system that utilizes data, quality improvement, education, and communities of practice to drive continuous improvement.

Duties

• Work alongside an operational leader in a co-leadership model to provide strategic, clinical, and system leadership for the Communicable Disease PCN.
• Collaborate with Saskatchewan Health Authority leaders on the PCN leadership team to establish a vision, build, and sustain a provincial Communicable Disease PCN.
• Co-create the PCN's shared purpose, strategic priorities, work plans, and project teams.
• Lead and participate in the development of standardized provincial approaches to communicable disease prevention, testing, case investigation, contact tracing, outbreak management, treatment, and linkage to care.
• Support the PCN community of practice through presentations, knowledge sharing, recruitment of content experts, and promotion of evidence-informed practices.
• Promote collaboration between public health, acute care, primary care, laboratory services, occupational health and safety, Infection Prevention and Control, Indigenous partners, and community organizations.
• Champion quality improvement initiatives, data-informed decision-making, and innovation that enhance communicable disease outcomes and system performance.
• Ensure health equity principles are incorporated into all PCN initiatives, with particular attention to populations disproportionately impacted by communicable diseases.
• Support the development and implementation of provincial standards, clinical pathways, educational resources, and patient information materials.
• Report administratively to the Deputy Chief Medical Officer for Provincial Primary Health Care.

Qualifications

The successful candidate will have:

• Has a special interest in communicable disease care of patients.
• In good standing with the College of Physicians and Surgeons of Saskatchewan (CPSS).
• Experience working across multidisciplinary teams and health system partners.
• Experience supporting evidence-informed practice improvement and system transformation initiatives.
• Strong communication, relationship-building, and stakeholder engagement skills.
• To be eligible, applicants must not currently hold employment or contractual commitments exceeding 0.8 FTE
• Experience working with vulnerable populations and a commitment to advancing health equity.
• An understanding of provincial communicable disease legislation, reporting requirements, and public health frameworks.
• Additional training and/or education in leadership/management would be an asset.
• Valid Class 5 driver’s license

The Communicable Disease PCN will focus on strengthening provincial coordination and consistency across all reportable communicable diseases. Early priorities may include:

• Standardization of communicable disease investigation and contact tracing practices.
• Improving access to testing, treatment, and follow-up care.
• Strengthening provincial outbreak detection and response.
• Supporting workforce education and communities of practice.
• Enhancing data, information-sharing, and reporting systems.
• Addressing rising rates of STBBIs and vaccine-preventable diseases.
• Building sustainable models for provincial coordination, and response.

Through partnership, innovation, and evidence-informed practice, the Communicable Disease PCN will support a coordinated system that improves population health outcomes and strengthens Saskatchewan's capacity to respond to current and emerging public health challenges.
