Job Description

Position Description:

Charlotte County Collaborative Wellness Centre (CCCWC) is seeking a Family Medicine Physician to join a collaborative care practice staffed by, nurses, LPNs, nurse practitioners, dietitians, social workers, respiratory therapists who work in a team to ensure patients receive primary care from the most appropriate provider. As a primary care family physician in a collaborative practice, the physician will deliver care appropriate to inpatient and outpatient services, work within an interdisciplinary team to provide family centered primary care which emphasizes health promotion, disease and injury prevention, chronic disease management and client education. The physician provides primary care functions to clients across the lifespan, and participates in the development, implementation, monitoring and evaluation of programs and services for individuals, families, and the community.


 

The physician’s principal role is to service the primary care needs of the CCCWC patients. All other clinical activities are secondary and are only available when the primary obligations have been met in coordination with the Physician Team Leader and the Clinic Manager. When additional work is performed during normally scheduled CCCWC time, the physician must indicate type of leave (e.g. education, teaching, vacation, unpaid leave). The Health Authority monitors FTEs and may recommend an FTE reduction to the physician if a pattern develops. This is not intended to be punitive, rather to optimize CCCWC patient access.

 

As a practitioner at the CCCWC, the physician takes a shared responsibility in ensuring that the site is covered during normal operating hours and that staff are aware of planned absences to ensure appropriate collaboration for nurse practitioners and family medicine residents. The physician agrees to the Accountability Benchmarks for Family Practitioners. Caseload is prorated if less than a full FTE.

Job Duties:

  • Perform clinical duties by assessing clients for routine health and illness care, prescribing treatment during scheduled hours of service, providing on call service when scheduled and/or making home/hospital visits as necessary, and by keeping other physicians informed with problems arising when on call.
  • Provide family centered primary care by taking into account risk factors (environmental, social, psychological and medical) in a manner which can be understood by the client, and when applicable, their families.
  • Prescribing treatment, consults, referral to other healthcare professionals as required and appropriate follow-up to patients across the lifespan.
  • Practice in a collaborative fashion as a member of an interdisciplinary team and consult with members of other health professions as appropriate.
  • Participate in the use of departmental and corporate evidence-based rules of practice to ensure that expected standards of care are achieved.
  • Participate in medical practice that is compatible with the Health Authority’s mission, values and strategic directions. 
  • Participate in the development, implementation, monitoring and evaluation of treatment, education, counseling and clinic health promotion, disease and injury prevention, and chronic disease management programs and services for individuals, families and the community.
  • Participate, by consensus of medical staff and in coordination with the Clinic Manager, in teaching and supervision of medical students and residents.
  • Participate, by consensus of medical staff and in coordination with the Clinic Manager, in teaching, mentoring and providing a resource to Nurse Practitioner students and other health care professionals.
  • Participate in after hours on-call coverage in coordination with other medical staff ensuring provisions are made for continual coverage of patient caseload during times when absent from the practice. 
  • The Health Authority and the physician recognize the right and responsibility of the Health Authority to manage its affairs and the responsibility the physician has to their patients as well as to the Health Authority, and both acknowledge that exceptional and unforeseen circumstances may occur when the Health Authority may need to call on the physician for help. Under such circumstances, within the confines of this Agreement and in accordance with the Scope of Practice and Position Description, the Health Authority, through the VP Medical, Academic and Research Affairs and Chief of Family Medicine may, in conversation with the physician and the Collaborative Practice, assign other professional medical work to the physician.

Note: These responsibilities are the basic and prime duties associated with this position; other related and allied duties may be assigned from time to time as may be necessary to the operation of the SJHCHC and Horizon Health Network.

Hours of Work:

Generally, hours within a 7.5-hour day will be dedicated using the following as a guide:

  • 6 hours minimum for clinic-based direct patient care or approximately 20 patients/day
  • in a flexible manner
  • 1 hour for clinical documentation
  • 0.5 hour of flexible time (clinical care and documentation and/or program planning, case conferencing, or other appropriate meetings deemed necessary by the CCCWC.

Inpatient coverage consists of two-week morning rotations for FTE physicians; or as required by practice.

Remuneration:

Fee for Service

Standard remuneration is Fee-for-Service which is a direct compensation between the physician and Medicare of New Brunswick. On call remuneration is provided. No source deductions can be provided.

Physicians may also opt for a salaried remuneration model.

Qualifications:

  • Must hold a valid license to practice medicine in the Province of New Brunswick
  • Must be a member in good standing with the College of Physicians and Surgeons of New Brunswick, and the New Brunswick Medical Society
  • Must have current malpractice insurance from the Canadian Medical Protective Association (CMPA) appropriate to the category of practice.
  • Certified in BCLS preferred.
  • Excellent communication, interpersonal and team-building skills.
  • Good attendance and work record.
  • Physically able to perform assign duties.

Incentives:

$100,000.00 accompanied by a 4 year return of service for eligible physicians approved by the Department of health.

Other Requirements:

  • Attend appropriate CPD events as required to maintain membership in the CFPC or equivalent to those requirements.
  • Participate in physician meetings and appropriate team meetings
  • Required to provide a three-month written notice of departure to the administrators of Horizon Health Network should the physician decide to leave the region.
  • Comply with the policies and procedures of the Department of Family Medicine

About the Facility:

Horizon’s Charlotte County Collaborative Wellness Centre (CCCWC) provides access to primary health care for the residents of Charlotte County.

Located on the third floor of Horizon’s Charlotte County Hospital , the CCCWC offers a wide-range of integrated services such as vaccinations, injections, well-woman visits and foot care clinics. The health care team delivering these services to the community consists of two nurse practitioners, a licensed practical nurse and administrative support.

About the Community:

Situated in the southwestern most corner of New Brunswick, bordering Maine. The area includes the large, populated islands of Grand Manan, Deer Island, and Campobello and the mainland communities of St. Stephen, St. Andrews, and St. George.
