FAMILY PRACTICE TAKE OVER FHO/FHT 

A well-established Family Health Organization (FHO) physician is seeking physician to take over an existing practice within a collegial group environment and a high-functioning Family Health Team. 

Practice Details 

• 1250 patients (higher percentage of geriatric patients). 
• 3 full clinic days with aprx ~20 patients/day – however, can gear towards your own preferences  
• Lease takeover $9,000-$11,000 monthly overhead inclusive to monthly lease, staff salaries, supplies, equipment and billing agent. Shared decisions with respect to hiring and expenses.  
• Support: Working alongside 3 other well established family physicians and well staffed clinic with 4 admin staff and NP  
• EMR: Telus PS 
• Clinic Hours: Mon-Thurs 8-4pm 
• After hours requirement 1-2 shifts/month, and a weekend shift every two months 
• Personal office spaces upstairs, along with a staff meeting boardroom and kitchen. Patient care and waiting rooms on the first floor.  
• 6 patient rooms, and one of those is a procedure room.  
• Parking is included in the overhead, and is very close to the office.

• CPSO-licensed Family Physician eligible to work in Ontario 
• Comfortable in an FHO/group model and participating in shared after-hours coverage
