Take Over a Solo FHO-FHT Practice that is Co-Located with 7 FHO Colleagues

A physician with a solo FHO/FHT family practice on the West Hamilton Mountain would like to retire and is looking for a successor to take over the care of her patients in 2026. The physician is looking for a candidate who wants to transition into the practice, starting with a 2-day/week locum for the first few months. This is an excellent opportunity for candidates looking to join the Hamilton Family Health Team while taking over an established, well-supported practice.

The successful candidate will be joining a large 23 member FHO and will be caring for a roster of approximately 1,600 patients (with an additional 200 un-rostered patients). With appointments typically booked at 15- or 30-minute intervals depending on the complexity of the patients, the practice typically sees between 15 – 20 patients per day. Current practice hours are 9 a.m. – 5 p.m. on Monday, Tuesday, and Thursday, and half-days on Wednesday and Friday, with on-call coverage rotated between FHO members. Up to 8 weeks of vacation coverage is provided by the FHO each year. 

This solo practice is located on the West Hamilton Mountain in the same building as a pharmacy and 7 FHO colleagues. The practice uses Telus PS Suite EMR and includes a waiting room, 4 exam rooms, 1 counselling room, and 2 private offices. Experienced administrative staff handle reception and patient bookings, and the practice also receives support from a psychiatrist as well as a variety of allied health professionals including a pharmacist, dietitian, and mental health counsellor.

Hospital privileges are preferred and will allow the incoming physician to access to Clinical Connect, CME, educational rounds, networking and to maintain or expand knowledge of the medical community and supports available in Hamilton.
