Geriatric Medicine Specialist (2 positions available)
The Opportunity
Peterborough Regional Health Centre (PRHC) is seeking Geriatric Medicine Specialists to join our growing geriatric program on a permanent basis. The role includes inpatient consultation, outpatient clinics, and home visits, all well supported by a team of experienced allied health professionals. Successful candidates will join a collegial team of seven Geriatric Medicine specialists, within a division that is actively involved in teaching Queen’s University Family Practice students. Locum opportunities may be available initially for those interested, as we currently have short-term coverage needs. As part of a truly continuum-of-care model, you will support older adults through our Geriatric Assessment and Intervention Network (GAIN) outpatient clinics serving Peterborough and surrounding rural communities, in addition to providing inpatient consultative services at PRHC. Your practice will focus on timely intervention, coordinated care planning, and safe transitions that uphold a “home first” philosophy and promote functional independence. PRHC is home to one (1) of the twelve Geriatric Assessment and Intervention Network (GAIN) clinics in the region. Our clinic plays a vital regional role in delivering specialized assessment, diagnosis, treatment planning, education, and interdisciplinary support for older adults experiencing complex and multifactorial health challenges. As a Seniors’ Care Centre of Excellence, Geriatric Medicine Specialists will have an opportunity to liaise with and support the Geriatric Assessment and Treatment Unit (GATU) and the Geriatric Assessment and Behavioural Unit (GABU). PRHC has a new Reactivation Care Centre with a “home first” philosophy. This unit is a participant in the Institute for Healthcare Improvement’s (IHI) Age Friendly Health System. Age‑Friendly care, by design. PRHC is one of the first hospitals in Canada to be recognized through the IHI Age-Friendly Health System - Committed to Care Excellence initiative, and our new Reactivation Care Centre is a showcase for embedding the 4Ms (What Matters, Medication, Mentation, Mobility) across teams and transitions. Help shape what’s next. We are currently redesigning our Geriatric Assessment and Treatment Unit (GATU) to fully align care practices with the 4Ms model. As a Geriatrician on our team, you will have a voice in co-design, implementation, and outcome evaluation. Additionally, we are planning to expand the 4Ms model organizationally, ensuring that its principles are integrated across our entire system. This will enhance our commitment to age-friendly care and support our goal of becoming a leading example of excellence in geriatric healthcare.
Position Qualifications
· Applicants must hold, or be eligible to obtain, licensure with the College of Physicians and Surgeons of Ontario (CPSO). A clear and viable pathway to licensure must be established at the time of application.
· Fellowship with the Royal College of Physicians and Surgeons of Canada (RCPSC) with Certification in Geriatric Medicine or equivalent training (e.g. American Board Certification) and eligible for RCPSC assessment.
· Strong interpersonal skills and ability to work effectively with physicians, nursing, staff, patients, directors, managers, and the community as required.
· Eligible for staff privileges at PRHC.
· Demonstrate the ability to integrate current evidence and best practice into the delivery of quality care.
· Evidence of ongoing continued medical education to satisfy the requirements of the appropriate college.
