Job Description
We are seeking a dedicated family physician to join the team, who is passionate about providing subacute care to patients who are in transition from acute care to the appropriate community setting. As the most responsible physician, the TCU physician plays an integral role in optimizing health holistically, managing acute and chronic disease, assist in the transition to the community, and preventing re-admission to acute care. The successful physician will have an affinity for leading the health care team in providing proactive medical care for patients and will develop expertise in complex discharge management support. We are currently hiring for January 2027, with potential to locum sooner if desired. The current rotation is 3 weeks on, 3 weeks off. The Subacute Hospitalist Program offers opportunities for expanding clinical skills, continuing professional development, and an unparalleled work-life balance in a supportive clinical environment.
Practice Setting:
• The Ottawa Hospital – Civic Campus • Transitional Care Units (TCUs) for patients who have completed their acute inpatient stay and are awaiting disposition planning
• EPIC EMR – full training provided The Ideal Hospitalist candidate will:
• Strive for excellent patient care • Possess great interdisciplinary collaboration skills
• Be Family Medicine (CCFP) trained
• Have subacute hospitalist experience
• Have excellent English language communication skills; French would be an asset
• Possess an independent license with the College of Physicians and Surgeons of Ontario (CPSO)
• Be registered in good standing with the CMPA
Work Expectations:
• Provide inpatient care for subacute medical and surgical inpatients
• Business day rounding 5 days/week (Monday to Friday) • Workload: 20-25 patients, with the expectation to be on-site for at least half a day each rounding day.
• Home call: each physician provides after hours and weekend home call coverage for 1 week every 6 weeks (divided Monday to Thursday, and Friday to Sunday), covering both the General and Civic TCUs.
